Applicants must submit a packet that contains the following documents:

○ The Completed Knights of Columbus Scholarship Application Form 

○ High School transcript posted through the most recent reporting period.

○ Letter of recommendation from one school official.

○ Letter of recommendation from a parishioner of St. Anthony de Padua (non-relatives);

○ Letter of recommendation from another non-relative individual- a family friend, co-worker, neighbor, or a supervisor.

○ An autobiographical description, in narrative format, which shall include hobbies, employment, scholastics, organizational activities, awards and major educational interest. (neat handwriting or preferred type)

○ Awardees must attend May 25 Mass at 12 for awards presentation after Mass.

○ Applicants must list all other Scholarships or Grants that will be earned this graduation. (amount not necessary)

○ I give permission to publish name and photograph award ceremony.
